I signed a conent for my PO that she forced me to so she can speak to them about my any & all info about me.?

About me, test results and to let them know I am on probation for a misdemeanor, and they should NOT prescribe me any pain meds even though I am having another major surgery. One Doctor has already discontinued me from his practice because of this. How do I retract or cancel this authorization with my Doctors to have her or anyone get any of my very personal medical info about me, let alone calling them and telling them I am on probation and why, etc...?
